Description: This toolkit describes a partnership between the Iowa Primary Care Association (Iowa PCA), Community Health Centers of Southeastern Iowa (CHC/SEIA), and the VA Office of Rural Health (ORH) Veterans Rural Health Resource Center-Iowa City (VRHRC-IC).
